When I converted our 30-year old tank water heater to a new on-demand model, I had a local fabrication shop make an l-shape transition collar. The outboard flanges were wide enough to cover the original opening. The inboard flanges were bent to wrap around the four sides of the new heater cabinet.
Essentially, this is what you need for your fan. I assume you would be using sheet aluminum for four project. I suggest tying the inboard flange to the roof channel around the larger fan opening. This will take the load off the top flanges on the roof.
This is really a no-brainer for someone who can bend the collar and weld the corners together.
